Okay, first let me concede the point that the Ace of Spades piece was technically "homophobic" insofar as shifting gears from "U R a wimp" to "U R a gay" was totally gratuitous -- I mean, he could've just elaborated on the wimp theme without dragging Greenwald's supposed sexuality into it.

But I don't know why Sullivan describes this as "unhinged" homophobia, implying that it's quite over the top and among the worst examples he's seen. I guess he's reacting to this:

Some of the comments on Matthew Yglesias' site (linked by Sully) are a hoot -- in a laughing at them way, I mean. Several posters are quite pleased with themselves to have caught AoS in an "error" about the location of the duodenum!

Frank, as I commented a little while ago on the Yglesias blog -- where everyone was falling over themselves doing the "Anal Sex Is Perfectly Valid But Don't You Dare Suggest That All Gays Enjoy It" two-step:

I notice that Ace of Spade's description of a vigorous cock-pounding was totally ambiguous as to whether the route of entry was via mouth or anus. (If you imagine a vertical line connecting the two orifices, the duodenum sits very roughly at the midpoint; on the other hand, if you consider the actual route that food takes through the entire twisting length of the digestive system, then the duodenum is actually much closer to the mouth than to the anus.)

In any case, the fact that AoS mentions the duodenum must surely prove that he's (a) prodigiously hung, and (b) straight as an arrow -- looking down, he observes that his own dick is long enough to reach the duodenum from other end, and having never seen another man's boner, he naively assumes that his is typical. QED.
